Then Satori wouldn't ever, ever, EVER lose. And the Touhou games would become a lot more... boring. Imagine watching a lunatic run. Instead of seeing millions of colorful bullets fly from everywhere, we'd have 3 hours long chess battles.

The plot is about Sakuya mastering her time manipulation power so that she can travel to the past in attempt to rewrite history to make herself the main character the series. This causes Reimu to meets her PC-98 self, so they team up to go through stages and fight bosses from each of the mainstream games, ultimately facing the maid herself in the end!

If "Outside world" meant "our world" I guess the first thing they will deal with is border police and customs, then visas... and probably international arrest and subsequent imprisonment. That is, if they did not decide to blow up/eat/damaku the hell out of the police first.

If we look positively, we will have a booming economy, and the Yokai will be on All-You-Can-Eat buffets in our world.
