Devil of Decline Installer help(Case closed)

Excuse me if this is something simple, but I just got Devil of Decline(along with Nightmare of Rebellion, but I haven't touched it yet.) and I kinda got a bump in the road with the installer. There's some info it wants me to give, I'm just unsure of what I need to put in. If you have the time and are willing, I'd really like to get through the installer.

Ah. I took one look at this and remembered I answered this in a previous Nightmare of Rebellion topic in 2016. It doesn't appear to have changed at all, so I'll just copy/paste from my previous post:
