The spellcards that you know you can do, but just barely. You're a pixel away from clipdeath the entire time, your heart is racing, and your finger hovers over the bomb button.
For me, Kogasa's midboss spellcard in Stage 2 (Hard/Lunatic). The bullets are so tightly packed, and since I never use Reimu, I'm always afraid of sudden clipping (which then leads to clipping, of course :V). Sakuya's Eternal Meek on Lunatic also does this, as well as (surprise!) Kogasa's first EX midboss spellcard.

Things I tend to fail in general, especially if I'm trying to beat something for the first time.
And definitely cards like Kogasa's "Bone-Dry Water-Repelling Monster" or whatever that was called. It can be hard to tell when you're about to get hit, especially if its going to be a clipdeath...

if you move up to the top to avoid most of the bullets then Kogasa will bodyslam you
move up there and then activate FROG NUKE for REAL SANAE DAMAGE
...or just stay at the bottom and smash the numpad like a lunatic while praying to god that you won't get hit

The attack starts off as a pretty solid stream down the center (move to the side), then bullets start to assault the entire central area from the sides (move back to the center and get off the bottom).

Your kidding me, I'd much rather fight marisa over reimu and her evil borders and nasty nonspells. Also marisa's theme and danmaku patterns are more awesome.

Oh, forgot, everything boss!Meiling throws at you on Lunatic. Colorful Light Chaos Dance in particular has killed me so many times that I basically have no confidence on it, even though I know how to do it :ohdear:

Oh I forgot to mention, but patchy's silphae horn or whatever its' called. That very first one she casts on Marisa A on lunatic. Seriously, I always have more trouble with that than emerald megalith, by far. I don't think I've capped it on lunatic once actually. It's scarlet meister hard for me >=P.

Really fast attacks.
Things like Kanako's opener and Evil Eye Sigma's second phase because you have to really pay attention and react fast.
PoFV and PoDD.
My first lunatic 1cc of PoFV is still one of the most intense shit I've ever gone through.
I had to take five after beating it because apparently I had forgotten to breath.

Utsuho's never fail to make me tense and shaking from concentration.

Border of Life and Death, Virtue of Wind God, Suwako's final, and the last phase of Hourai Elixir. Basically, really dense or really fast stuff, especially if a clear depends on that card.

Attacks with bullets that come from the sides or from the bottom, and attacks that force you to move horizontally through the pattern instead of just normally getting through gaps (Scarlet Meister, SA pelletspam, etc.).
Also, Kaguya's night spells. "oh lol hi you've got a pretty nice score there, but if you don't ace all five of these spells right now then you get completely screwed, kthxbai"

The last 20 (or is it 30?) seconds of a Subterranean Sun timeout.
The last ten seconds of Suwako's second timeout card.
The last five seconds of Rising World.
THE FUCKING TIMER COUNTDOWN SFX DURING A VOWG TIMEOUT.
Boundary of Wave and Particle, StB edition (SA edition is a pansy).
These aren't the hardest things out there, but rather those that require constant movement on the part of the player or naturally build up tension during the course of the attack.

Practically everything in UFO except Murasa.

Horizontal bullets. Fortunately these are very rare. Not-quite-horizontal bullets are fine, though.
Also, Subterranean Sun, because of the X-axis pull when you move away from the exact center. It throws me completely off, most of the time I don't even know what hit me. It's the only card in the series where that still happens.

Um... Seven Signs of the Deva, that card of Youmu is one I can barly capture, but still can, and it definatly scares me.
Doumen Seimen, kinda... I haven't captured it yet, but I can tell I am close. Stupid bullet-course changing thingie D:<

This comes up quite a bit. The difference between Marisa and Reimu in Stage 4 of Imperishable Night is the difference between micrododging and streaming. I can stream like it's an Olympic event, but micrododging just murders me. I'll take Reimu every time over Marisa's ridiculous cascade of tiny tiny spinning stars.
I have wondered what these definitions mean, so streaming would be run away from the evil thing coming at you, and micrododging as it suggests just slowly nudging around through gaps, right? (this is probably on the wiki, isn't it?)

Almost any attack in which there are bullets coming from left and right at the same time. Like Suwako's lilypads or Youmu's opener.

Something like that, but for what I have seen and heard, Streaming is slowly moving from 1 side of the screen to the other, slowly enough that your grazing, but fast enough that your not dying. This is used on attacks where the enemy fires at your current location, such as Kaguya's Fire Rats Robe.
Micrododging is slight left and right movements, going through the gaps. That 1st Yuyuko non-spell that has the fan-thingie out would be a time to micrododge, I think.
