Because i have no friends.
Its because i enjoy it. There is little in this world that beats the satisfaction of doing something awesome in Touhou. It can be timeouts of certain spellcards, beating extras, hard modes, lunatic modes, no-focus stuff. Everything have its own degree of satisfaction to it because it gives me a feeling of having surpassed my own limits.
Another thing that keeps me hooked is the music. There is many shmups out there but for some reason, its only Touhou i like. Music plays a big part of that. While other shooters in my opinion have bland and uninspired music that's only good for a few listens, Touhou music never seems to get old for me. I have even spotted a tendency that i play the stages which music i enjoy more than other stages. For example, i have played Stage 3 in SA over a 100 times but in MoF i have only played Stage 3 about 60 times or so because i like the music in SA S3 more.
Finally, i'm hopelessly in love with Touhou girls. That's probably the largest factor.

The huge fanbase always comes up with something I'll enjoy, stare at drooling, or back away slowly questioning wether or not getting into the fanbase was a good idea. Gameplaywise, the music, the crazy difficulty, and all those little characters that I drool over adore.
Also the fact it was made by one drunk dude.

Music yes, the music is great.
But my main reason is the difficulty, I love hard games. I love trial and error gameplay unlike some people and just doing the same thing over and over untill you get it just right.
I also like the diversity, out of all touhou games which look similar? Sure there are some spells that have a resemblence but each boss and stage is unique and requires it's own approach.

- using your bombs to convert masses of bullets into faith items in MoF (honestly, I'll have an orgasm -- not literally, but you know what I mean -- if I ever get to reach Garegga's Black Heart mkII's bomb spray phase)
- gathering lots of items into the UFOs (if you know me for long enough, you may have heard of my greedy side when playing shmups. How would gathering lots of items into a UFO that multiplies their value.. NOT attract me, like a shark is attracted by the scent of blood?)
honestly, I dislike grazing, but have recently played Shikigami no Shiro 3, and enjoyed grazing and collecting coins with multiplied value! Could it be that this game appeals more to my greedy side, by allowing me to stick near bullets that would be considered "already grazed" in Touhou, and directly rewarding me with a multiplier after killing enemies while grazing, instead of forcing me to graze as much bullets as possible?

It's fun, and I had never played a shooter or fighting game before Touhou, so...
I dunno. The fan created stuff is funny, so I keep coming back for more.
And besides, my older "Anime Must Burn In Hell" sibling (who, ironically, showed me most of this stuff in the firstplace) is overseas.

But everything past EoSD is really fun to play. Awesome bullet-patterns, nice backgrounds to some characters, really great music, and the doujin/music out there makes it all even better.
Only thing I dislike is some fan interpretations of some characters *cough*Alice*cough*

I initially started playing after I listened to a brofistful of awesome songs...that and mcroll got me started. Not to mention a friend of mine showing me a brofistful of lunatic replays. The insanity got me started and then I played IaMP for a bit. I was really bad at it. Roughly few months ago I tried out a few of the main games and got completely hooked. Touhou has been among the hardest games I've ever played. Which made finally beating MoF that much more satisfying.
A few minor points
All the crazy lucky dodges and the stupid deaths.
Wow, I haven't even got to the brofistful of assimilations the fanbase has done over the years. It's gotten to the point that my friend calls touhou the zerg. I no longer see that as a bad thing.
As for the fact that touhou has a mostly single gender population. I jsut remember that the creator is drunk almost all the time. Needless to say figments of a drunk imagination will get drunk themselves. I think I just like to make alcohol jokes.

This, to a certain extent. The fanbase can get 'tarded sometimes *cough*Sanae hate, pad chief maid fanboys*cough*; and not even the best remixes can top ZUN's music.

I eventually found a video of EoSD Extra and went "This shit is insane I could never do it." and that was that. Well, it sunk in for a few days and drove me mad until I finally got EoSD and played it. Oh the lulz that were had. I couldn't even finish Easy Mode with all 3 continues. Hahaha.
Anyways, I play Touhou for the challenge and sheer fun of it. The amazing music and hilarious characters keep me coming for more. It's also fun to see people's responses. "Holy shit that's insane how can you see to do that." :V

This. Also Shmups were always one of my favorite genre. Touhou finally made a shmup what is PERFECT to my taste.

I enjoy a good challenge, and I've always been a fan of shmups. Touhou feels challenging, but not impossibly so. The huge number of great fanworks really helps to define the world in my mind, to say nothing of the awesomeness in canon (see: everything but ZUN art).
Also, the porn.

Aside from the music, danmaku, characters, and overall charm that they give the series, I think one reason I love playing Touhou is the difficulty. It's been a really long time since I've played a "solo" game that was this hard, and a part of me loves to take on these challenges and push my limits. It feels so good when you find yourself in the zone and pulling off shit you never thought you could.

I don't want to say the difficulty, since that's more like a frustration to me, but... There's something really great about the rush danmaku gives that's totally different from any other type of game. It's extremely hard, but surprisingly enjoyable.
Of course, the music and characters became major reasons quickly after I started playing.

Awesome games, tinkly dramatic piano music, pretty fanart, lolis, etc. Y'know, the usual stuff. I used to like the fanon aspect too, but I've pretty much completely lost interest in it aside from the aforementioned fanart.

Because Touhou is the most fun I've had playing a video game in quite some time. I like challenge and there is pretty much endless replay value.

Even as a child I quite enjoyed shmups. I never recalled any type at the time that had people as I mostly played the air craft types (Gradius, Cyvern etc.)
So what motivated me to pick touhou up was really the different experience that it delivered as a game. This time the storyline was more focussed, people were fighting, the music and danmaku patters are considered.
To my delight the fan response and commmunites for Touhou were pretty big too, so it's much like a family appreciating the different elements of the game and it's supporting fan works ;)

It's a great game. It excells in every catagory of gaming that I care about pretty much:
music, challenge, replayability, Characters (story isn't important to me often, but the characters often are, and most Touhou characters are definately loveable). I really like the whole goofyness to the whole thing as well. Also in particular for a shooter game, I like them to feature pretty bullet patterns, which Touhou is among the best of.
The fan-made products for Touhou are jaw-droppingly good as well. Labyrinth of Touhou for example is quite possibly one of the best RPG games I've played for me, the challenge yet balance the whole way thru, with no crap wasted on meaningless extras and overpowered optional gear. I hate now RPGS these days slap boring as @%#%# goals to get that 100%, and think that's proper replayability (zomg let's make the palyer dodge 1000 lightning bolts in a row in this area for ____'s ultimate weapon!).
The music, I really hate it when people say none of it is as good as Zun's. IMO that's the opinion of someone who really doesn't love the music. I mean I respect ZUN, and I think he deserves every bit of credit he gets. But he only writes his music Zun-style. Sometimes you want something rockier, or with metal, or maybe you like vocals. His style doesn't offer that. So even if you prefer that style over metal or something, I wouldn't call it "better", or "worse", but "different". They take his awsome compositions, and re-make it to fit people who want to listen to it with a different style, and no matter what, the results are often pleasing.
I don't mean to come across as a mindless fanboy. But seriously, you can take pretty much anything, and Touhouificate it, and it often suits my tastes better due to inserting a (most likely) cooler cast, great music, and often increased challenge (yet more balance).

The thrill of weaving through the beautiful patterns of the spellcards declared towards you, the player. When you're done and finished, you throw your hands in the air in delight that you have beaten the game!
That's why I play Touhou. The characters and the music just made it even better... icing on the cake, as they say.

My main five reasons.

Play it? The only one I can even halfway play is SWR and UNL, and currently only successfully 100% on Hard. Can't quite pull off Lunatic just yet.
No, it's mostly the musical arranges, the massive fanon world, the adaptive storyline (Read the little extra bit in the doujin 'Why did she leave the miniature garden', from black!Tewi for a better understanding) and the huge cast.

Anyhow, for me its because it gets me engaged in much more things than just the game.
I want to draw better since I see so much cool art now, I want to learn music since listened to so much ear-candy, I want to learn more because I want to know the origins of the characters. ... and so much more :)

I heard the game was difficult and I wanted to figure out how the games played.
Now it's because I have a lot of fun playing it and I like to complete challenges with myself. Though sometimes I have to make sure I do the challenges correctly. Because at one point I had the last two stages of EoSD slowed down to 50 FPS, I repeated the entire playthrough to make sure I properly 1CC'd it with 60 FPS throughout the entire thing.
Also I play the fighters because a couple of my friends got the fighters as well, so we've been playing against each other with it. I'm also the only one of the three of us who can actually host games. Though it can end up becoming very competitive and we sometimes keep playing until around 3 AM in the morning.

The Music (Got some of the song stuck in my head), Its challenging, the characters, the memes it has spawned, and games that they parodied(ex. Touhouvania).

There are many games I enjoy, many of them shmups. What makes Touhou special to me is Gensokyo.
The creator has taken the time and effort to write literal books on the backstories, personalities, and inter-personal relationships of a fairly large cast of video game characters, even writing about the history of the world they live in. This, to me, breathes a magical life into the games - it makes me care about the characters.
That dimension on top of an already well-made shmup sets Touhou apart.

Touhou is a series of games I've consistently enjoyed and it is also a far cry from my steady diet of RPGs. It's a great change of pace that takes all of 30 minutes to enjoy if I manage a 1cc run. I've thoroughly enjoyed the music as well, though I seem to be enjoying the (good) remix groups moreso than the actual game music recently, which is quite an accomplishment.

Why do I play Touhou?
Well, not everyone likes a damn hard game with a heavy Asian undertone full of "crap flying all over the place"
But it's fun. I love difficult games. I love the music. I also like the storyline, however vague it may be. Something about not giving up and bragging rights as well.
Especially comments from people who say, "How the hell do you even dodge that!? I thought you were supposed to absorb the bullets! That's impossible!"

However, let's not forget fandom. I knew Touhou was a game from the start, but the Chirumiru video is what got me to start keeping an eye on Touhou, each time staring closer. It was a cute little video, a little weird at first becuase I didn't understand anything, but I liked it. It was catchy.

The massive supply of fanon, the whole nature of the 'Make it your own' behind Gensokyo's backstory and the like, and the fact that it's got a huge already-established character cast, which the fandom has a field day with. Because I dont find many games as intense to play as touhou. It severely makes me concentrate and work very hard for my victory, so its very hard but its not impossible. Feels massively rewarding when youve just come out of a spellcard with no scratches when you think you should have died already. seriously makes me sweat to play lol. works up a "danmaku musk" I call it :V
Also music and atmosphere helps, touhou PCB and UFO are great for both imo. Yet to play PC-98 touhou, but im getting addicted to mima's theme, so i'm gonna have to at this rate.

Saw some videos on Youtube, and the music sounded nice, and the characters are interesting, that, and the games looked within my ability to play, sort of, just no precise platforming, or insanely long button combinations that you have to do in about two seconds to pull off, or something.
The fanbase looks interesting too.

I'm a very casual shmup fan. The only recent games I've played in the genre, other than Touhou stuff, are some of Kenta Cho (http://www.asahi-net.or.jp/%7Ecs8k-cyu/index_e.html)'s freeware games (which are pretty cool). Likewise, I've so far been a casual Touhou fan: I own copies of PCB and IN but haven't played anything more recent, although Double Spoiler looks interesting and I'm considering having it be where I start officially following the series. (Of course, as a player whose crowning achievement so far is just barely 1cc'ing PCB Easy, Double Spoiler will probably murder me, but oh well.)
That said, despite the fact that I will probably never be an exceptionally good player, I'm interested in getting better at Touhou because I'm fascinated by how much Internet love there is for these games. Can't stand/get enough of ZUN's artwork? Here's thousands of pieces of fanart! Love affair with the game soundtracks? Here's thousands (probably) of remixes and arrangements! It's completely nuts, and I can't help but be drawn to something that inspires so much creativity (memes aside) from its fans. That said, I do think it's important to keep a handle on the original work as opposed to just being swept up in the fan stuff, hence my desire to follow the games themselves.
And umm, well, as a gay guy, the concept of hundreds of magical girls flying through the air while smack-talking and shooting beautiful rainbows of death at each other's faces is too ridiculously pretty to not be hooked by. :ohdear:

I just got into MS recently. I gotta say, It's become a fast favorite of mine. Yumeko has won over my heart.
