Well...I'm still trying to improve my game, haven't been able to finish a Touhou game yet on Normal...but so far, what I got are:
1. Wriggle

EX: Not skilled enough to reach an EX-boss yet

In my generic and non-controversial opinion:
1. Yamame
2. Chen
3. Alice
4. Patchouli (MarisaA)
5. Orin, or Yumeko. It's a tough call.
6. Remilia
EX. Evil Eye Sigma (Educated guess, since I've never actually played SoEW extra. Though I'm guessing its difficulty comes more from shitty game mechanics than the actual danmaku)

4. Aya(normal and up)
5. Tie between Sakuya(for irritating hitboxes) and Sanae(for holycrap amounts of danmaku at once)
6. Out of all of them, I have the most trouble with Eirin. But out of actual difficutly, Remilia.
EX. Never actually beat an EX boss, so I couldn't say. Only EX bosses I've actually gotten to are Ran and Suwako.

6. Good question. Remilia is harder from a pure skill perspective, but Kaguya is much more hateful and made of BS and argh. So I'm giving it to Kaguya, but not for good reasons.

1 - Wriggle. Just Wriggle.
2 - Parsee, who is amusingly hard for a stage 2 boss. I'm jealous.
3 - Alice Margatroid, especially her Foggy London Dolls.
4 - Reimu, of whom I still dislike. Honestly, Reimu, I hate you.
5 - Honestly, Orin. I've never made it to Sanae for some reason, and all the other PC stage 5 bosses are really just weakened by their own ability. I swear, there is not a time in which Sakuya's timestop gave me time to figure out a good path to take, Youmu slows down the game for me to easily dodge incoming bullets, and Reisen just kept making her bullets easier to pass through by making them untangible.
6 - Yuyuko and her butterflies of DOOM. Forget Kaguya and her annoying Imperishable Night's End, those butterflies now scare the crap outta me everytime I see them. Remilia kills me too quickly for me to know, as I can't play Normal well, and Utsuho, though laughable to you, is just below Yuyuko for me.
EX - Despite not completing Normal mode on anything other than the fighters, I seem to have completed enough of a few extra stages to conclude that Flandre is my hardest bout. Mokou is tough, and so is Ran, but the hitboxes are at least visible on PCB and IN.
Fighters - Patchouli. No matter how hard I try, she is the MAIN reason I fail at SWR's story mode on anything higher than Normal mode. Other reasons include Yukari, Reimu, and a hint of Iku.

I'm only counting bosses in the Windows Touhou games. i haven't got the best memory of the PC-98 bosses (at least how hard they are)

From what I can remember...
1. Yamame. Lunatic Yamame. Way worse than any other Lunatic Stage 1.
2. Parsee. Lunatic Parsee. FFFFF. And maybe Mystia or Kogasa very close behind.
3. Keine. I just can't do it. She's so unremarkable too. But she just bowls me over halfway through any IN run - worse than Marisa or Reisen!
4. Reimu. Maybe Satori or Aya on a bad day come close, but definitely Reimu.
5. Youmu. Reisen and Sanae are actually pretty easy, and Sakuya and Orin are at least bosses that I can find the way to deal with, even if they're still sorta suck to me. Youmu just runs me over in Yukari's train.
6. Not sure. Probably... Flandre or Yuyuko? I haven't played 6 or 7 since being a Touhou novice though...
6-Final Attack. Utsuho's Subterranean Sun. They're all hard, but at least I can follow them and expect to maybe bomb once or twice, Subterranean Sun drove me through two lives and about two-three extra bombs to spare. It hits the halfway point and I just crack - already stressed out and crosseyed from freaking Heaven and Hell Meltdown. [Kaguya's EoIN cards would count but you don't need to clear em].
EX. Koishi Komeiji. I'm not sure how hard of an extra boss she is... because it's damn ridiculous just to REACH her. Alternately, Evil Eye Sigma because that crap is ridiculous - even in the remake, which uses the exact same bullet count/size/ratio, but actually gives you far more precise control.

EDIT: Under further evaluation I've come to think that Yuki and Mai are by far the hardest 4th stage boss(es) and that Remilia is a lot easier than I last remembered.

1-> I'd say Letty or Yamame but the one who every once in a while takes me by surprise is Rumia... I RAGE every single time O.o
2-> Again there IS Icicle Fall, which I absolutely dread, and Chen has some really nasty spells, but Parsee just takes the cake.
3-> Yuugi. Plain, simple and uncontested.
4-> I see a lot of Ayas back there, but for me she's not so bad after a goddamn hell of a stage. BoWaP makes me cry almost as much as Non-Drectional lasers from Patchy but DAMN YOU REIMU!!!! DAMN YOU TO HELL!!!!

Well, I'm not sure about Hard/Lunatic (because I suck too much for them still on most of the games ;_;), but since I've at least beaten every stage on Normal + Ex...
1. Yamame. Especially on higher difficulties from what I've heard/seen.
2. Meira, the way bullets fly out of her sword slashes is a bit unpredictable. Kogasa deserves an honourable mention too.
3. Yuugi, by a large margin.
4. Yuki and Mai. Patchy is also a problematic stage 4 for me.
5. Orin. The mechanics of her spellcards can be very strange and I always suck horribly in the fights no matter how much I practise. ;_;
6. Does Yumemi count? >_> Otherwise I might say Shinki... if only because I failed miserably to 1cc because of her when for fun I played a normal run (AFTER I'd beaten EX-Alice no less). Remi's probably got technically some of the hardest stuff to avoid.
EX. Evil Eye Sigma, hands down. Even without the game's lack of a hitbox and weird game mechanics (compared to later games that is), some of her attacks are extremely fast and hard to avoid due to unpredictability. The 'bouncing balls' attack is probably my vote for hardest attack to survive in any EX stage. Strangely though, if you manage to beat the bouncy balls, every single one of her attacks afterwards is ridiculously easy by comparison. Seriously, it's the only boss I can think of that gets easier once her health gets low enough!

1. Wriggle Nightbug in Imperishable Night (you know how it tastes in Lunatic mode)
2. Kogasa Tatara in UFO (nice pattern)
3. Alice Margatroid in Perfect Cherry Blossom (Spring Kyoto Dolls is troublesome)
4. Satori Komeiji in Subterranean Animism (she is the one with... so many spellcards and they are not easy...)
5. Orin in Subterranean Animism (need to move around and has long life)
6. Remilia Scarlet in Embodiment of Scarlet Devil (simply cannot do it)
EX: Suwako Moriya in Mountain of Faith (who can beat her raining and 2nd last spell?)
Conclusion: Subterranean Animism is the WINNER!!!

My Normal Difficulty analysys:
- Minoriko Aki is easy to screw up during her regular attacks and on her last spell card, if you don't know what you're doing. Nazrin's only hard if you fail at the Nazrin Pendulum. I'd have to say that Minoriko's harder.
- Definitely Kogasa Tatara. Too easy to screw up all-around.
- Ichirin Kumoi. I swear, it's because of Kumoi and Tatara that I can't consistently 1LC the TH12 demo. >_<
- Something of a toss-up between Reimu Hakurei, Marisa Kirisame, and Satori Komeiji. I've played against Marisa more in IN, but IIRC, Reimu may be a little easier to master. And Satori is, well, Satori. She can use Yukari's attacks against you. That's scary. Almost said Aya Syameimaru for this one, but decided that these other three ladies were tougher.
- Rin Kaenbyou. No question. Orin is that one boss (http://tvtropes.org/pmwiki/pmwiki.php/Main/ThatOneBoss) for Touhou.
- I'm gonna have to go with Eirin Yagokoro or Kaguya Houraisan for Stage 6. I don't think anyone gives me trouble quite like they do.
- Based on the Extra-difficulty videos/replays that I've watched, Yukari Yakumo and Koishi Komeiji get this one. Good grief, I must be some kind of masochist or something. I wanna get to them, but I know they're gonna destroy me.
