With each mainline game good-end cleared at least once, I've been trying to branch out into the unofficial games, but boy are there a lot of them.
To (slightly) narrow my scope:
1. No danmakufu
2. Not purely a puzzle game
3. Has at least basic menu translation (commands or a FAQ)
So, aside from the Koumajou Densetsu games (already played because **** yeah Castlevania), what would you suggest as the best/your favorites? Comprehensive mods can count.

Gotta pimp my Diablo 2 mod, Lord of Maidens. Thread here (http://www.shrinemaiden.org/forum/index.php/topic,14331.0.html).
It's not perfect, there's a few bugs and balance issues but it's playable.

If you're in an RPG mood, go for Genius of Sappheiros, Labyrinth of Touhou, or Touhou Pocket Wars. There's other non-RPG fangames, but I only really know about the RPGs, so... yeah.

You could also try Gensou Shoujo Taisen (Fantasy Maiden Wars) for a really good SRPG or Touhou Rhythm Carnival for a really good rhythm game.
There's also the classic Marine Benefit and both Suwapyon games if you want another shmup.

Labyrinth of Touhou and Genius of sappheiros are amazing rpgs in their own right that blow most commercial ones out of the water in terms of strategic depth and replayability IMO.
Mystical chain is a really fun game. It's about as long as a regular touhou game, but its playstyle is more like..umm.. some kinda single player adventure version of super smash bros? It's kinda hard to explain. Just watch some youtube vids of it, they capture the gameplay pretty well, and are pretty fun to watch.
I know you said no danmakufu, but I dunno why. Maybe you only enjoy them to be full games instead of individual stages or minigames? If so I really like phantasm romance. Its danmaku is even prettier than the official games IMO, though the music is nowhere near as good.
Everyone else seems to prefer concealed to conclusion though, so you may wanna try that instead if you're open to trying anything at all. I didn't think it was as fun though personally.
I still haven't gotten around to trying that Youmu Ys game, derp.
Also if you like pokemon (I never tried it), Touhoumon is supposedly amazing, I dunno.

I said no danmakufu so I'm not overwhelmed by choices (for now), though yes, I do prefer full games.

Touhou Soccer 2 is a must, especially with friends, where it turns into ridiculous contests of epic proportions. I mean, alone, it's just... Kinda game breaking after a while. Fun, but y'know, use x powerful shoot, pray it goals, repeat. With live friends on the same computer? It becomes crazy. It's like an eternal duel between MASTER SPAAAARK and China Boobs-Wiggles.

Touhou Rhythm Carnival Scarlet .
It is an EoSD rhythm game.
There isn't an English patch as far as I know, but if you are familiar with the rhythm heaven series it should be easy to figure out.
Edit: I just remembered there is one danmaku level, but its rhythm based. I promise.

If you're into visual novels, I'd recommend the "For Busy Person" series. Five games (counting the Extras) have been translated thus far, and they're very fun, albeit long, reads. Full voice acting, too, though I think they pulled random people off the street instead of using famous VAs like Koumajou Densetsu. No matter, they still do a good job voicing the characters.

Megamari (Mega Man style game), Takkoman (feels like a cross between SSB and Mega Man. With yukkuris), Labyrinth of Touhou (dungeon crawler RPG. Good but very grindy), Youyou Kengeku Musou (that Youmu Ys game that Ghaleon mentioned), Mystical Chain (stages start to repetitive really fast but the bosses are fun).

Doggone it, I worked for almost a year and a half to translate Aya Shameimaru: Touhou Attorney and none of you rubes recommend it?

Seriously that game holds a special place in my heart for combining the two series I love the most in the whole world.

If you makes you feel better, I kind of wanna try it now because I'm a fan of the Ace Attorney series (...and Touhou, herp).

I dunno about everyone else, but I absolutely adored it. I just felt like it captured the essence of the original game, as well as terrific plot-twists, like every PW games. Plus, the translation is amazing and flawless. ;)

Gensokyo no Nazo is pretty fun (megaman-like platformer) but it's pretty hard.
Did a normal mode run last year and as of an hour ago I just finished a lunatic run (finished for me = all characters obtained, not visiting all 220 stages, I'd rather stab my eyes than find them all).
edit edit: Just keep in mind if you do play this game, make sure to set the folder to be able to be written in, else the game will crash if you beat a boss/save).

I wouldn't say it was flawless but I tried to make it as natural as I could. Hopefully the second will be even better (once I get around to it).

For starters, you should go check Tasogare Frontier's games, being the co-op group of ZUN's Shanghai Alice in the fighting games, they make great ones, such as:

PatchCon - RTS style Defense game
DynaMarisa 3D - Earth Defense Force inspired game
Satori's Educational (something something) - isometric puzzle game starring Satori guiding Koishi to the goal via arrows and items
Area Zero's Touhou Sky Arena (3D fighting game)
UTG Software's Dangerous Suwako Chan - SMB inspired (hard mode plays much like IWBTG)
Desunoya's Fantasy Explorer Nitroid - Metroid inspired game starring Nitori
Snow Gale's Mugen Nise Sato - Valkyrie Profile inspired. Seems to be part one of a series
Blue Mica's Remi Ojou Doki Doki (Remilia's Heartfelt Adventure or something) - a puzzle platformer
Sanae Challenge Fusion - a Platformer game with Sanae and Okuu
Youyou Kengeki Musou (a personal favorite) - 3D action adventure game starring Youmu
Hachimitsu Kumasan's Sakuya Counterattack - Arena style game starring Sakuya
