Actually, I think those requirements aren't accurate. I just played through normal and 1cc'd with three bombs in stock. I got ending 3 for it. Hmm...

That's just what the blog said, and I'm not willing to test what the exact requirements are since I've already played through enough times just to see each ending once.

Nah. It unlocks a second loop, but only if you cleared with only one death and less than two bombs as well.

Looking on ZUNs twitter, he seems to be working hard on some of the bugs right this moment, so they are surely going to be fixed soon.
I only got so much out of the google translation, but it seems quite clear.
