Her second to last spell card with all the anchors is often an auto-bomb for me. :/

first Koishi doesn't like the fact that I'm going to capture Danmaku Paranoia, so she pushes me all the way to the side where I clip something trying to turn around

then I clip Subterranean Rose 3 times with full bombs

I can get to the 30 second mark (When Mokou spawns like 5 concentric rings).

That's probably the hardest part; if you can manage that you're basically through (provided the green waves don't panic you). You want to be at the left hand side of the screen for the first set and the right hand side for the second set so that you're in a position to get through them by moving upwards. Don't get twitchy and try to fly through those gaps really fast; take your time and be accurate. Even when the circles start closing, you have enough time to make it through, the bullet hitboxes are smaller than they look. :V

Eh, I did the second 5-ring set by entering sideways from the left. Probably not the best idea now that you said you can enter by moving upwards from the right but for some reason I never feel comfortable in the bottom right corner (go figure).

Then again it "only" took me 59 tries so I'm not really complaining.

I'm trying to get a better score than my current one(544m) and almost improved it a bit.....got 6k graze, decided to use my remaining bombs on Subterranean Rose to milk it for more graze.....and just as it ends, I die on the last second.

Except for one stage death, no deaths until her survival, even timed out Genetics without dying/bombing......then suddenly 2 deaths on Philosophy(both extremely stupid) and one on Subterranean Rose..... I forgot to go up to the middle during the second phase.
Naturally, I perfect the rest of the card.

There's no point in grazing Subterranean Rose.  Grazing only increases the value of point items by 1% for each 100 graze, and there aren't any after Philosophy.  On the other hand, you're going to get 40 million points for each life left and 8 million for each bomb (or 2 bombs for Marisa A) left at the end, plus ~8 million for capturing Subterranean Rose.

LLS Lunatic, game overed in the SAME PLACE (yeah, the whole 99.83% complete), 11 deaths again...but only 26 bombs.

5 bombs.

It really doesn't help when Yuka moves TWO-THIRDS OF THE WAY DOWN THE SCREEN BEFORE DROPPING THAT ONE CLOUD OF RED AND BLUE BULLETS WITH NO ROOM TO DODGE.  Or when Marisa does pretty much the same thing.  Or Elly.  Or Orange.

Damn it, I am absolutely useless against Yumemi. I tried PoDD Normal, no deaths until Stage 9, everything was pretty easy. I then die four times in a row to Yumemi. :V I never have any room to move away from the homing crosses because I get clusterfucked by everything else at the same time.

Also, inability to figure out how to do IN's stage 3 with Scarlet Team/solo Remilia. I mean, what the hell. Stuff comes from pretty much everywhere so you can't just park Remilia's familiars somewhere for more than three seconds without it backfiring, and Sakuya's "spread" shot is a joke, I mean, Reimu's needles anywhere else in the series have a wider shot, besides, y'know, KILLING stuff for a difference.

I gave it like 50-60 tries, then gave up and decided to never touch it again.

Then I randomly captured it one day in an otherwise horrible Extra run.

It'll do that.  Your replay isn't corrupt or anything, it's just that SA has a tendency to desync right at the beginning of the Utsuho fight.
