Mad Dance is...absurdly simple...to put it nicely.  Go watch any replay...

...you know what?  This should do.  Kanako moves towards you, so if she's feeling particularly mean she can move around in funky distances (like moving way past you so you can't hit her for a wave or two), but all you need to do is exactly the same thing, every single time.

On topic:
Falling Leaves of Madness: 140/204.  64 insta-ragequits right there.
Autumn Skies and a Maiden's Heart: 61/102.  That means around 40 more restarts in between there, and then 40 more on this card.  I REALLY REALLY hate MoF Stage 1.
Mad Dance: 131/148.
Divining Crop: 71/142.  That record remains at exactly half no matter what I do.
VoWG: 1/137.  Goddamnit.

My history for Recollection: DBDB and Possessed Fairy are both like 3/150 or something ridiculous like that.  I don't know why, but I never got any good at either of them, no matter how hard I practiced.  I also have perfect records on both Genetics of the Subconscious and Rainbow UFOs, despite having never cleared either SA's or UFO's Extra stages (They're both 2/2 :V).

Ten Evil Stars:  14/15.  The one failure was on a failed would-be first 1cc run that failed because I lost my last spare life here.
Hell's Tokamak:  13/15.
Subterranean Sun:  9/15.

I also have probably over 500 attempts at Fujiyama Volcano across all shot types because I love screwing around with that card for no reason.

Mysterious Snake Show, SanaeB:  38/41.  HOW DID I FAIL THIS THREE TIMES?!
The only UFO Lunatic cards I have captured are Busy Rod, Hello Forgotten World, Sinking Anchor, Complete Clarification, and Nirvana's Cloudy Way in Purple (WHAT).

Fantasy Heaven: 1/401 (yeah, fuck this card)
Blazing Star: 1/84 (didn't even bother using focus...there was just...no time for me to use it)
Phantom of the Grand Guignol: 1/86 (thank god I pulled this off in less than 100 attempts...this spell card nearly made me lose faith in humanity...I swear)
Profound Danmaku Field: 1/154 (I'll admit - I had to watch vids and replays of this spell card...if it weren't for that, I'd probably end up 1/300 or something)

PROTIP: No horizontal makes this card absurdly easy; just completely ignore the fast curving streams of butterfly bullets, and only pay attention to the slow ones.

My solo Reimu record is 0/348(all of these being failed timeout attempts). My record for actually trying to capture is 3/85 with Reimu and Yukari.

I also have a low cap-rate of Heaven's Dragon, mostly because the card is tedious and you get full power after it anyways, so bombing it is not disadvantageous at all unless you are playing for score, which I am not.

Now for something more important:

VoWG lunatic: 1/39, though if my PC hadn't barfed some time ago, it would be 1/1xx...

Actually browsing through my histories, it seems pretty much all of my last word captures are also gone due to the barf, as well as several other things. 
The one thing I vaguely remember, though, was the history of THE CARD:

The one that makes me rage is Busy Rod Lunatic with SanaeB: 223/256. It's not the capture rate itself so much as I've faced the card over 250 times and I'm not done yet arghhhhhhh
