Well, I actually haven't really paid that much attention to the regard of some characters' ears being possibly fake in general, but I remember to have seen some rather rare instances of Momiji drawn without ears.
Because ZUN has neither drawn an official portrait for her so far nor she's gotten one in any spin-off release, I think the fanbase just goes with her general animal species and therefore mostly draw wolf ears, which also applies to certain official
print works.
About Reisen's ears: I've seen already quite a number of comical pictures on Tumblr where the buttons on the bottom of her ears are heavily emphasized or they're just being entirely taken off by herself or by Tewi without depictions of any injuries whatsoever, implying that they're just accessories and she uses her human ears. I was initially utterly confused about that when I just joined the Touhou fanbase, but now I've just settled with her rabbit ears being actually real and the buttons being accessories. A rabbit without rabbit ears would look kinda awkward, wouldn't it? Just like above :V
