Install issues: "Miracle Super Party -Sanae and Tenshi's Illusion Labyrinth-"

So I recently purchased Miracle Super Party -Sanae and Tenshi's Illusion Labyrinth- during the last convention I was at, but all my attempts at installing it have met in resounding failure.  As far as I've been informed by more kanji-literate people, the following slide on the installation screen has 4 input boxes, each input box referring to User ID, Email, Password, and Password retyped.

Now, whenever I input the information as I've been informed to do, I get an error which translates to "There is a mistake in the inputted information. Please reenter it."

Now, inside the DVD Box was a slip of paper. I've tried having my friend who can translate some Japanese make heads or tails of it, but they couldn't figure out what it was saying. The album of images I uploaded to imgur is located here: http://imgur.com/a/tys4e#0
I'm not quite sure what I'm doing wrong with installation, if someone could shed light on what I'm doing wrong it would be appreciated greatly.
And if this is in the wrong section, can someone please move it to the right area?

Quick edit: I'm also running on Japanese Locale, so I don't need to change locales to Japanese or run AppLocale, or I don't think I need to. The operating system is Windows 7 Home Premium.

The instructions don't seem to have anything special in them. Plop the disc in, write in your info, input the serial key in the next window. The info you put in will be used later for support and authentication.

Whoops, my bad! Removed the former album and reuploaded the images without the serial key being part of it.

It's odd because it sounds more like there's a typo in your info. There doesn't seem to be any actual verification going on at this step, so it isn't as if you put in wrong information.

EDIT: It's said that if you input a User ID or email already in use it will be rejected, but that doesn't seem to be the problem given your choice of name.
EDIT: Can only use english half-width characters for User ID. No problem there.

I can't fathom what I'd be typoing, save for passwords, but I'm making sure the passwords are correct in both instances every time.
