Wait a minute.. How could i forget about Marine Benefit - the best unofficial touhou fangame i've ever played? Or it's just so awesome, everyone counts it on par with official games? :V

Now that I think about it, Rain of Blood over Elysium is pretty badass.

I forgot about Lovely Visitor from Hell. I do like that one, but my favorite is still Tempestuous Second Sun.
Some runners-up include Rain of Blood over Elysium and Wind from the Thankless Land. :V

Finally back to Touhou, so I got a lot to catch up:

Most difficult Stage 6 Midboss: 6A Eirin, I can't do anything she throwes at me(yet). But Sakuya is a close second place.

Favorite "death fairy": EoSD Extra, the "wall-fairy" before Patchy.

Favorite Stage 4 boss: Patchouli, I like pretty much all of her spellcards.

Best final card: Flying Fantastica.

Favorite nonspell: Yuyuko's nonspells.

Favorite stage portion:PCB stage 4 :yukkuri: MoF, Stage 3.

Best final spell of an extra stage: Ripples of 495 years (it's the only one I've seen personally...)

"Best" Touhou choke: Kaguya final spell in IN Normal. 3 SECONDS left Also Double-KO on Flying Fantastica in UFO Easy a while back.

Favorite stage title: Frightening eyes no one loves.
