Title: How do you get extra lives in Mountian of Faith?

Besides getting extends from bosses how do you get extra lives? I'm trying to beat Kanako, but I can't kill her with only two lives. :(

From memory: 20, 40, 80, and 150 million points.

Also, a life drops in Stage 3 and 5.
